Baylor Scott & White Community Care – Carrollton is devoted to the total health care of uninsured and under served people of Carrollton and its surrounding communities.  We provide continuing medical care for individuals and their family members of all ages, from children 5 and older to the elderly.  

Our expertise is based on the knowledge of our patients in the context of their community.  This community has a very high incidence of uninsured residents, many of which suffer from chronic diseases such as diabetes.  Our primary focus is on providing a medical home to those residents that are in need in order to assist in their overall health and wellness.  ​

Baylor Scott & White Community Care – Carrollton offers comprehensive care for patients of all ages. Our physicians and staff are able to provide a variety of health services allowing patients to receive all of their health care needs within our office.

We welcome new patients and can often accommodate same day appointments.



Making an Appointment

On your first visit, we will begin a permanent record. Please bring any medications you are taking, so that we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form. Please bring your insurance card and try to arrive at least thirty minutes early for your first visit. If you have been under the care of another doctor, we may ask your permission to transfer some of your records.

We have three appointment categories that take up varying amounts of time:

Annual Exams or New Patient Visits – Plan on spending one hour including check-in time, filling out forms, and the actual physician visit.

Follow-up Exams – Thirty minutes

Sick Visits - Varies depending on the illness, plan on at least thirty minutes.

CHIP and Children's Medicaid

CHIP and Children’s Medicaid both cover services needed to keep kids healthy, including:

Dentist visits, cleanings, and fillings

Eye exams and glasses

Choice of doctors, regular checkups, and office visits

Prescription drugs and vaccines

Access to medical specialists and mental health care

Hospital care and services

Medical supplies, X-rays, and lab tests

Treatment of special health needs

Treatment of pre-existing conditions

If a child getting Medicaid has a disability or long-lasting illness, we will find out if the child can get special services.

Children’s Medicaid is free

If we find your child can get Children’s Medicaid, there is no cost to you for service.

CHIP fees vary based on your income

If we find your child can get CHIP, your enrollment fee and co-pays will be based on your family’s income. Enrollment fees are $50 or less per family, per year. Co-pays for doctor visits and prescriptions range from $3 to $5 for lower-income families and $20 to $35 for higher-income families.

Available opportunities are medical assistant intern, student nurses, health information, health promotion/marketing/social media, and computer programming. Hours are Monday - Thursday from 9 a.m. - 5 p.m. and Friday from 9 a.m. - 1 p.m. A background check will be required.

Medicaid and CHIP

Children's Health Insurance Program (CHIP) offers low-cost health coverage for children from birth through age 18. CHIP is designed for families who earn too much money to qualify for Medicaid but cannot afford to buy private health coverage.

Medicaid for Adults

A person:

 Either: (1) is age 65 or older, or (2) has a disability that is expected to last a year or longer.

 Needs 30 or more days of non-stop, long-term care.

 On SSI (Supplemental Security Income)

 Has little or no money.

 Doesn’t own or is not paying for items over a certain value. (Items that aren’t counted are homes, vehicles, limited amount of burial funds, personal belongings, and some types of life insurance.)

 Have a physical, intellectual, developmental, or mental disability.

 Are working

 Don’t live all the time in a nursing home, state hospital, or intermediate care facility for people with intellectual disabilities.  


Cost

A person who gets Medicaid for long-term care must pay part of the cost by first using all his or her own money, except for a small monthly amount for personal needs and costs like health insurance premiums.  

People that “buy-in” to Medicaid coverage by making monthly payments.

If you have Medicaid coverage, there is a Medicare saving program that helps pay for some or all the Medicare monthly payments.

PediPlace is a non-profit pediatric healthcare practice for children from birth through eighteen years of age who are uninsured or who qualify for Medicaid or CHIP (Children’s Health Insurance Program) benefits.

PediPlace is designated by the State of Texas as a site serving a medically underserved population. We provide a medical home for children that tend to be from low income families who otherwise would go without care or who would unnecessarily use the emergency room.
